Jose L. Ruiz, M.D.
Before it was a cosmetic treatment, BOTOX was used to treat medical conditions related to overactive muscles. Today, it's used by many physicians to reduce symptoms such as chronic tension headaches, migraines, excessive sweating, overactive bladder, and more. BOTOX is trusted by doctors and patients all over the world for its versatile aesthetic and medical indications.
The human face has more than 50 muscles. Because BOTOX removes wrinkles by preventing muscle contraction, practitioners must know all about the underlying facial muscle anatomy and know which facial muscles to treat with BOTOX in order to achieve the best results and avoid complications. Dr. Gross has the knowledge and experience to ensure your safety, create natural results and maintain a normal level of facial expression when performing BOTOX injections.
BOTOX Cosmetic With a Facial Plastic Surgery Specialist
If you have sun damage, scarring, or areas of excessive pigmentation on your face, your BOTOX injections can be combined with a Fraxel® laser treatment.
The primary difference between BOTOX and other injectable treatments such as JUVÉDERM®, JUVÉDERM VOLUMA™ XC, RADIESSE®, or Restylane® is that BOTOX is not a facial filler. Rather than filling in or plumping up wrinkles to sit flush with the surrounding skin of our Orlando patients, BOTOX works by limiting the movements of certain muscle groups in the face. These include muscles in the forehead (frontalis), in and between the eyebrows (corrugator), surrounding the eyes (orbicularis oculi), the muscle over the upper nose (procerus), circling the mouth (orbicularis), and at the corners of the mouth (depressor anguli oris). Dr. Gross also treats neck sagging and bands by treating the thin muscle that drapes from the jaw and down the neck (the platysmal muscle).

How Much Does BOTOX Cost?
At Primera Plastic Surgery, as well as most other practices, BOTOX is priced by the unit. A "unit" is simply a fixed quantity of the product. There are 100 units in each vial of BOTOX. Although the cost per unit is fixed, each patient requires a different number of units to achieve his or her desired results. Therefore, patients with more significant concerns pay more than patients who wish to treat fairly minor wrinkles. In general, a BOTOX Cosmetic procedure at Primera Plastic Surgery costs anywhere between $300 and $600. During your consultation, Dr. Gross will give you a quote that reflects your specific needs and desired results.
A Smooth Recovery
Unlike more invasive cosmetic procedures, BOTOX injections create minimal discomfort. We also offer nitrous oxide (laughing gas) to reduce discomfort during this treatment. Nitrous oxide is a safe, short-acting analgesic similar to that used in sedation dentistry and preferred by many patients. The procedure itself is extremely quick, lasting only a few minutes. For many people, a BOTOX appointment is easy to fit into a workday or an afternoon of errands. Although BOTOX doesn't require any sort of downtime or healing, it's important to avoid bending, lowering your head, or manipulating your face for the remainder of the day. This allows the product to distribute evenly, and these restrictions don't typically interfere with most daily activities.
Immediately following your treatment, you may experience some symptoms at or near your injection sites. These are typically mild and can include redness, swelling, itchiness, tenderness, or bruising. Typically, these symptoms resolve themselves within a few days.
BOTOX results appear in 5 to 7 days and can last for 3 to 6 months or more. Repeated treatments often prolong BOTOX results.
"Face muscles are complex, and a thorough understanding of contracting and counterbalancing muscles is needed. This relationship must be understood before injecting BOTOX."
– Dr. Gross

Proven Techniques, Outstanding Results
BOTOX Cosmetic is an FDA-approved substance that is injected to minimize the appearance of wrinkles in both the upper and lower face. To understand how BOTOX works, it's important to understand how wrinkles work.
BOTOX Cosmetic treats "dynamic" wrinkles, which are caused by repetitive facial movements. These include crow's feet, which are caused by squinting or smiling, and glabellar lines, which form between the eyebrows and are caused by furrowing your brow or frowning. Of course, we can't selectively avoid making facial expressions forever just to reduce the chance of developing wrinkles. This is where BOTOX Cosmetic becomes useful. The active ingredient in BOTOX temporarily relaxes the muscles that cause these wrinkles, reducing their appearance and smoothing the skin.
Dr. Gross is an experienced injector and conservatively administers BOTOX Cosmetic precisely where it's needed. At Primera Plastic Surgery, he uses BOTOX to treat common aesthetic concerns that include:

Orlando’s #1 Quality BOTOX Injector!

On the right a 42 year old woman has her “11 lines” (glabellar lines) virtually eliminated while frowning. Patients are shown 2 weeks after treatment. Getting Rid of Your Wrinkles, Smile and Frown Lines and Those 11’s!
Unfortunately we all get older. It’s a fact of life that we cannot change. But just because we age doesn’t mean we FEEL OLD. But increasingly noticing wrinkles on our faces sure doesn’t help. Around our mouths with smile and frown lines, and of course the 11’s on our forehead. Fortunately BOTOX was innovated by ophthalmologist medical research. And it works! When in the hand’s of an experienced injector, BOTOX is an extremely reliable solution to address facial wrinkles.
Yes, An Expert Physician Injector Makes a Difference!
Does it matter who your BOTOX injector is? Absolutely! People often make the mistake of choosing a BOTOX provider solely on price. That can be a big mistake however. At YMD Eye & Face, Dr. Yeilding does ALL filler and BOTOX injections. Additionally, Dr. Yeilding is a board-certified facial plastic surgeon. And during her intensive fellowship training in oculoplastics and facial plastic surgery she conducted a complex study that compared BOTOX to another alternative botulinum toxin treatment. So what does that all mean to you? Unlike many low-cost BOTOX providers, because of Dr. Yeilding’s extensive medical training and experience, she has an extensive knowledge of the complex human facial anatomy, and an advanced injection technique that results in the most natural outcomes.
BOTOX has deep Ophthalmologist roots – including Dr. Yeilding.
California Ophthalmologist Dr. Alan B Scott, while a scientist in the late 1970’s for Smith Kettlewell Eye Research Institute, did the first FDA clinical studies using botulinum toxin in humans. He published his work in the Transactions of the American Ophthalmological Society in 1981 showing safe and effective results to relax eye muscles that caused strabismus (misalignment of both eyes- sometimes appearing as crossed eyes, or one eye turned out). Allergan Inc purchased Dr. Scott’s company and attained the first FDA approvals in 1988 for what was renamed BOTOX, for treatment of strabismus and blepharospasm (when the eyelid muscle goes into spasm). It was another Ophthalmologist, Dr. Jean Carruthers, who was conducting a BOTOX blepharospasm study, accidentally discovered the first cosmetic benefit of BOTOX when she found her study patients had reduced glabellar frown lines. Dr. Carruthers went on to publish a large body of work pioneering the use of BOTOX Cosmetic, including defining the scientific methods for assessing cosmetic efficacy. In Dr. Yeilding’s clinical study, she used the Carruthers methodology and validated grading scales to assess patient results for glabellar lines, forehead lines and crow’s feet.
Aging Thins our Skin and causes Skin Laxity (stretching) but Muscles make the Wrinkles
As our faces age, one of the most common physiological changes is the thinning of our skin while also increasing skin laxity (stretching). When another common aging effect happens, a decrease in fat pads of the face with age, we find that gravity makes the skin sag. For saggy, thinning skin attached to very active muscles, the repetitive muscle movement causes the skin to bunch up and we see wrinkles. Some of the most common repetitive muscle contractions are smiling, frowning and squinting.
Three common wrinkle problems due to muscle contractions are:
Crow’s Feet
Forehead Lines
Glabellar Lines (the 11’s)
Botox®, The Gold Standard in Neuromodulator/Neurotoxin
BOTOX® is the gold standard neuromodulator/neurotoxin (also known as neuromodulation). Introduced in 2002, BOTOX® Cosmetic was a revolutionary product for the treatment of moderate to severe glabellar lines (those 11’s between your eye brows). BOTOX® has also recently received FDA approval for crow’s feet. Since 2002 multiple uses of BOTOX® have been shown safe and effective, including treating strabismus, blepharospasm, hemifacial spasm, hyperhidrosis (excessive sweating), cervical dystonia, spasticity of elbow, wrist and finger muscles, chronic migraines, urinary incontinence, crow’s feet wrinkles and more. Dr. Carruthers compared BOTOX® to Penicillin because it was so versatile against a wide range of conditions. Cosmetic uses grew very quickly because it had very apparent results, does not require surgery and has essentially no recovery time. Over 11 million BOTOX procedures have been done in the United States alone.
How does BOTOX® Work?
Using a fine needle, Dr. Yeilding injects BOTOX into precise muscle locations without requiring anesthesia. The treatment is usually completed in about 15 minutes in our Orlando office. By acting on the neuromuscular synapse, BOTOX reduces muscle contraction and therefore the wrinkles that go with it. This effect is not permanent however but does last 3 to 4 months depending on the patient.
How fast is the BOTOX® recovery?
Within minutes any effects at the injection sites are typically gone, certainly within an hour. There is essentially no recovery time with most patients. Sometime there is slight bruising but this is rare with Dr. Yeilding’s gentle injection technique. The full effect of BOTOX is in 2 weeks after treatment however Dr. Yeilding finds her patients receive significant benefit by day 3.
Dr. Yeilding of YMD Eye & Face chooses BOTOX® for her patients
Ruth Hill Yeilding MD, Medical Director of YMD Eye & Face, Winter Park, FL, in the heart of the greater metropolitan Orlando area, has perfected the use of this reliable product, BOTOX Cosmetic for wrinkle reduction. Dr. Yeilding has super specialized training, first in Ophthalmology, to gain understanding of the ultra complex set of muscles, nerves and blood vessels around the eye and face, and then Oculoplastics and Facial Plastic Surgery to be able to effectively manipulate facial anatomy to achieve aesthetic beauty. During her fellowship with nationally known facial plastic surgeon, John P Fezza MD, Sarasota FL, Dr. Yeilding conducted a complex clinical study comparing BOTOX to an alternative botulinum toxin treatment. Dr. Yeilding brings a deep knowledge of facial anatomy, an advanced gentle injection technique and a total commitment to personal patient care in her role as expert injector of BOTOX.

Is BOTOX® the right choice for me?
There are many different types of wrinkles we acquire with age and the details turn out to be highly patient specific. All patients are different yet almost every patient does seem to have at least ONE of the issues above. During your consultation with Dr. Yeilding she will access all of the anatomical elements- fat pad position and size, muscle tone, muscle movement, skin thinning, skin laxity (stretching). For patients with glabellar line (the 11’s), forehead lines, crow’s feet or frown/laugh lines around the mouth, Dr Yeilding will determine whether BOTOX is the right choice for you.
Dr. Yeilding’s Customized Treatment Plan for YOU
Based on her detailed assessment, Dr Yeilding will carefully create a custom treatment plan that considers your age and aesthetic goals. Sometimes when there is more than one issue, Dr. Yeilding will advise you to first tackle that issue that can be helped the most with a single treatment. This way the patient can evaluate how the results feel to them over time. Most of Dr. Yeilding’s patients are thinking long term, and many receive multiple treatments over time, some more aggressively than others, but Dr. Yeilding will restrict the amount and scope of treatments to always yield conservative but effective aesthetic enhancement. Knowing when to say no, verses doing a little more is one of Dr. Yeilding’s key attributes. Patient wellbeing is always first with Dr. Yeilding.

Where did BOTOX® come from?
The most interesting thing about Botulinum A Toxin (BTX) is that, in its undiluted and natural form, it is a potent poison – a neurotoxic protein that is produced by the clostridium botulinum bacterium. If ingested by a human, BTX can cause a life-threatening type of food poisoning, called botulism. However, in its commercially refined and purified configuration, BTX, better known as “Botox,” it is a useful drug that doctors employ as an effective treatment for many different types of health problems.
Approved as a muscle relaxant by the federal Food and Drug Administration (FDA) in 1989, BOTOX® was originally used to treat facial spasms, eyelid spasms (blepharospasm), the muscle spasms that cause strabismus, or crossed eyes, and cervical dystonia (a neurological movement disorder). BOTOX works by blocking the release of acetylcholine, the neurotransmitter that the brain sends to a muscle, commanding it to contract. When a targeted muscle is no longer getting the body’s normal electrical/chemical stimulus to contract, formerly uncontrollable spasms are reduced or eliminated.
Over time, physicians began to administer BOTOX® for several other medical conditions, including:
Cosmetic use for BOTOX® treatments
Because of its ability to halt muscle contractions, in effect, causing temporary muscle paralysis, Botox began to be seen as an effective neuromodulator for aging skin and a way to temporarily improve the appearance of moderate-to-severe “frown lines” between the eyebrows (glabellar lines). When underlying facial muscles don’t contract, the overlying skin remains smooth and unwrinkled.
After producing encouraging results in formal, clinical trials, the FDA approved the use of BOTOX® for cosmetic use in 2002. In addition to frown lines, BOTOX® can also help reduce “crow’s feet” and other fine lines on the forehead and around the lips and eyes – lines and wrinkles that tend to accumulate over time as a result of years of smiling, frowning, and squinting.
Today, BOTOX® cosmetic providers, in addition to plastic surgeons, include dermatologists, aesthetic spa physicians, dentists, nurse practitioners, nurses and physician assistants. However, results can vary widely based on the medical background and experience of the provider. According to the American Society of Plastic Surgeons, there were approximately 6.3 million BOTOX® procedures performed in the United States in 2013. The global market for botulinum toxin products is forecast to reach $2.9 billion by 2018.
The BOTOX® Procedure
BOTOX® is a safe and non-invasive procedure that can offer its users a more youthful appearance without surgical intervention or long recovery periods. In fact, a typical procedure usually lasts only ten minutes. The treatment is injected into the muscle one to three times via a very fine needle. Minor side effects are rare, and may include a slight redness or swelling at the injection site(s) which normally diminishes very quickly. Following treatment, visible results of BOTOX® Cosmetic are usually noticed within 3–5 days. The maximum benefit will be reached within one or two weeks.
A single treatment lasts about three months. The effect fades gradually, but since it can be repeated indefinitely with no lasting harm, a patient can return for treatments three or four times a year. However, BOTOX® should not be used during pregnancy, while nursing, or in combination with certain medications.
Before having a BOTOX® procedure, consult with Dr. Yeilding about any of the following you might have taken r

The muscle relaxants BOTOX®, Xeomin, and Dysport, and the fillers Restylane, Radiesse, Juvederm, Voluma, Belotero, Prevelle, Perlane, and Sculptra are injectable treatments designed to address certain signs of aging such as fine lines, wrinkles, and the commonly associated loss of volume seen in the aging face.
One or more of these products are also often used to maximize the improvement of surgical procedures, putting the finishing touch on the benefits of a facelift, laser treatment, or eyelid surgery. BOTOX® has also been proven to treat excessive sweating, although the improvement is not as long lasting as with a miraDry treatment.
Maximum improvement is often noted when Botox is used in conjunction with a filler, with the Botox relaxing the muscle that created the wrinkle and the filler softening its appearance.
Whatever methods are used, injectables are very effective in reducing lines and wrinkles and adding volume to your face. We will review your individual goals as well as your skin’s condition and your facial shape to establish the best treatment program for your needs.
About Botox
BOTOX®, Xeomin, and Dysport (collectively called “neuromodulators) are all variants of a highly purified and refined bacterial toxin that paralyzes muscle tissue, and are used to safely and effectively relax the muscles into which they are injected, temporarily diminishing or eliminating their contraction. They are specifically useful in treating those muscles that are used for smiling or frowning (called “muscles of facial expression”), eliminating the muscles’ ability to contract and make the wrinkles. All 3 of these products are made from a highly purified, FDA approved, cosmetic form of botulinum toxin.
In aesthetic medicine, these neuromodulators are often used to improve “dynamic” wrinkles- meaning those wrinkles that are caused by the facial expressions we make to communicate with others. The art of their injection, and what comes with skill and experience, is in creating the right amount of relaxation so that the patient remains attractively expressive, while minimizing the formation of these wrinkles. The most common areas treated with Botox, Dysport, and Xeomin are:
Frown lines between the eyebrows
“Crow’s feet” wBotox rinkles on the sides of the eyes
“Bunny lines” on the top of the nose
In these areas of the face, contraction of the facial muscles just under the skin causes a repetitive creasing of the overlying skin. When we are younger these lines occur only with expression. However, over the years, there is a quicker breakdown of the collagen in these hyper-mobile areas and the lines begin to persist at rest. These “dynamic” wrinkles therefore deepen with age and often create a tense, stressed appearance that makes some of us look older than we would like or feel. People may comment that you look tired, worried, or angry, even if you are feeling refreshed and in a great mood.
By treating the muscles causing these wrinkles with neuromodulators like Botox, Xeomin, or Dysport, relaxation and diminishment or complete effacement of the wrinkle results. The overlying skin looks smoother, tighter and younger. If done correctly most people feel that they still look like themselves, but “more relaxed.”
Treatments are very simple to perform and usually take less than five minutes per facial area. Using a tiny micro-needle, a minuscule amount of one of the FDA-approved neuromodulators (botulinum toxin type A), is injected very precisely into the select facial muscles, according to the individual patient’s goals and examination.
Because the needle used for the injection is very thin, no sedation or local anesthetic is necessary. Comfort can be enhanced by applying a topical anesthetic or an ice pack a few minutes before the injection. There is no “downtime” associated with these injections and patients often return to work the very same day that they are performed.
The results are usually noticeable about 3 days following the procedure, but reach their maximum effect within two weeks of the original injections. Although results vary, most individuals can expect significant wrinkle reduction for about four months following the injection of Botox, Xeomin, or Dysport. To maintain the desired wrinkle reduction effects, treatments can and should be repeated about every four or five months. The result tends to be longer lasting with subsequent treatments.
Your satisfaction, as well as the duration and effectiveness of the treatment you receive will depend to some extent on your biology, the product used, and the skill and experience of the person you choose to perform your treatment. At Aesthetic Enhancements, we are very fortunate to be able to offer you the services of Bridget Kelly, PAC, one of the nation’s top experts on injecting neuromodulators and fillers for aesthetic improvement. Bridget has been honored by both the manufacturer of Botox, and Restylane as a master injector and has participated in the training of many other professionals interested in improving their injecting skills.
What type of results will I see with BOTOX?
Typically, the effect of BOTOX is visible within 3 days. Maximum benefits are reached between 10 to 14 days. Once the muscle becomes dormant and the skin softens, you will have a more relaxed and youthful appearance. BOTOX treatments are not a substitute for the results achieved with a facelift. The optimum appearance may be achieved with BOTOX therapy in conjunction with a facelift, laser resurfacing, and skin rejuvenation.
What is BOTOX?
BOTOX is a therapeutic muscle-relaxing agent derived from the bacterium, Clostridium Botulinum. For over a decade ophthalmologist have applied BOTOX for its original intention, the treatment of blepharospasm. Physicians have utilized BOTOX for years as a cosmetic treatment to correct visible signs of stress and aging.
How does BOTOX work?
The contractions and movements of the facial muscles are controlled by signals from the brain. BOTOX simply prevents the muscle from allowing responses to these messages. As a result, the muscle relaxes and the overlying skin remains unwrinkled and smooth.
What is a BOTOX treatment like?
With a very fine needle, BOTOX is injected into the specific muscle that is contracting and causing the lines and wrinkles on the face. Treatment is effective, safe, and takes approximately 10 minutes. The pain is a minimal "pin prick" and no anesthetic is required. Immediately following treatment, you may resume regular activities! Treatments can be performed at our Sebring and Hunters Creek (Orlando/Kissimmee) Florida locations.
How Much Does Wrinkle Correction with Botox Injections Cost?
Wrinkle correction with Botox Injections Cost ranges between $132 - $550 in total, based on the individual's needs. Actual costs for Botox Injections will be determined once a complete evaluation has been performed during your consultation.

How long does a BOTOX treatment last?
The effects usually last 3 to 4 months, and fade away gradually. Following several treatments, the results may last 4 to 8 months or longer. BOTOX offers extended relief over the course of long-term treatment. Treatments are performed at our Sebring and Hunters Creek, Orlando/Kissimmee, Florida location.
Are there any possible side effects of BOTOX?
Side effects of BOTOX, if any, are usually mild and temporary. Botulinum toxins contain proteins, which in some circumstances may be naturally safeguarded against by the body's ability to produce antibodies. If antibodies Are formed, the therapy will generate a lesser response. Temporary bruising is sometimes experienced, though this symptom resolves itself within 2 to 3 days. In rare cases, there can be a slight drooping of an eyelid or asymmetry of expression. If this should happen, return to your physician.
